  4.     I have the primary right to make my own decisions concerning
  5. treatment that might unduly prolong the dying process. By this
  6. declaration I express to my physician, family and friends my
  7. intent. If I should have a terminal condition it is my desire that
  8. my dying not be prolonged by administration of death-prolonging
  9. procedures. If my condition is terminal and I am unable to
  10. participate in decisions regarding my medical treatment, I direct
  11. that my attending physician to withhold or withdraw medical
  12. procedures that merely prolong the dying process and are not
  13. necessary to my comfort or to  alleviate pain. It is not my intent
  14. to authorize affirmative or deliberate acts or omissions to
  15. shorten my life rather only to permit the natural process of
  16. dying.
